Good decisions can be made using a combination of intuition based on experience, and data. We will make decisions differently depending on if we have to do so very quickly or if there is no specific time limit. Who we involve in decision making can often dictate to what extent the implementation of a decision will be successful. This template form guides you to consider and doxument many of these questions.
